This information sheet should be used by members when applying for a Flexible Work Arrangement (FWA) and by QNMU members who are line managers who are presented with a request by a nurse or midwife for an FWA. It pulls together the key material from a number of QH resources (policies, guidelines, a checklist and guides) into one document.
