Lexi Shah - Navigator, Queensland Children's Hospital

Lexi is a Nurse Navigator with Navigate Your Health and a member of Children’s Health Queensland Senior Nurse Collaborative Group, Statewide Child Protection Clinical Partnership Steering Committee, Statewide Paediatric Community of Practice, and Statewide Nurse Navigator / Midwifery Navigator Network. She cares for and directs the systems that facilitate wellbeing for children and young people in Out of Home Care or on Youth Justice orders.

As a senior nursing leader in her team, she has developed a Navigate Your Health (NYH) Onboarding Booklet for new Nurse Navigators, Peer Assessment Tool and undergraduate nursing clinical objectives guide for nursing students. Lexi has reviewed and rewritten the NYH Operational Guidelines, contributed to the Memorandum of Understanding between CHQ, Child Safety and Youth Justice, Business Planning Framework, funding submissions, service planning, and reviewing pathways. She completed the Accreditation documentation in line with the NSQHS standards for NYH.

To ensure the care of some of Queensland’s most vulnerable children, Lexi has established inter-agency partnerships across primary and tertiary care providers including Aboriginal Medical Services, Child Safety service centres, Youth Justice service centres, Primary Health Care clinics and other hospital and health services. Lexi is committed to a strength based, person-centred approach when delivering care coordination, empowering client’s self-care and self-determination strategies to improve outcomes. She negotiates with clients/partners and seeks to establish goals which align with an individual’s needs, providing holistic management plans for the individual and the system around them. An advocate for clients, encouraging informed choices and supporting them with their individual needs/goals, Lexi respects differing beliefs and actively seeks to ensure the delivery of safe, respectful services to clients as well as supporting members of her team. 
 
Recently she received a Certificate of Appreciation from the Western Districts Youth Justice team for providing education to young people attending the Transition to Success Program.
